​Thank you for your participation!

Showcase your wonderful products at our fundraiser there are a few things we need from you first. Once you have completed this form and submitted ALL of your paperwork, you will receive a confirmation that we have received all of your information.

All vendor donations are tax deductible and all proceeds from the fundraiser go directly The Friends of President Polk's Birthplace Inc., a 501c3 organization. The Friends of President Polk's Birthplace, Inc. annually funds summer camps and field trip experiences for children, special events and guest speakers.

1 - Fill out our Vendor Application Form

2 - Facilities Use Agreement (Polk site staff will supply this for you after we receive your form, vendor donation and the rest of your documentation)

3 - Obtain a Certificate of Registration from the DOR via Business Registration | NCDOR

4 - Provide us with a copy of your Certificate of Registration and Tax ID Number
​
5 - Food Vendors must possess a valid Temporary Food Establishment Food Vendor Permit from Mecklenburg County.  

6 - Submit your $20 vendor donation.
